AB Signal

Clinical intelligence for HCC risk adjustment in Medicare Advantage.

SIGNAL, powered by EchoSignal

SIGNAL reads your charts and clinical notes and suggests the conditions the evidence supports, for your providers to review. Every suggestion carries its evidence and an audit trail, and all of it stays inside your environment.

What does SIGNAL do?

SIGNAL works alongside three teams in your organization, starting from what is already in the chart.

  • Risk adjustment teams

They receive the conditions the evidence supports but the current year does not document, each one validated against MEAT.

  • PCPs

They receive a paginated report grouped by PCP, with every finding presented as a recommendation with its evidence. The provider decides.

  • Compliance

Compliance has the evidence behind every finding and an audit trail that stays inside your organization's environment.

What problem does SIGNAL solve?

The problem is not fraud, and it is not inattention. It is missing documentation: a condition the patient demonstrably has, with evidence already in the chart, is not documented again in the current year, so it disappears from risk adjustment and the plan is paid as if the patient were healthier than they are.

SIGNAL finds those conditions that are already present but undocumented, validates each one against MEAT, and presents it as a recommendation for the provider to decide.

How does SIGNAL work?

SIGNAL works from evidence your providers have already recorded. Every finding comes back as a recommendation with that evidence attached.

  1. SIGNAL reads the clinical evidence already recorded in your group's EMRs.
  2. It compares that evidence against the CMS-HCC V28 model.
  3. It detects conditions the evidence supports but the current year does not document.
  4. It validates each one against MEAT: Monitor, Evaluate, Assess, Treat.
  5. It presents every finding as a recommendation with its evidence tree, in a paginated report grouped by PCP.

What does SIGNAL add to a risk adjustment program?

Capture more RAF

SIGNAL finds what the patient demonstrably has but was not documented this year. Each condition reaches the provider with the evidence already in the chart, so it does not quietly fall out of risk adjustment.

Keep what you earned

Every finding arrives with its evidence and its MEAT validation, so what the provider documents is built to stand up in a CMS audit.

CMS has announced that it will audit all eligible Medicare Advantage plans each year in all newly initiated audits, approximately 550 MA plans. Read the CMS announcement

Why can compliance trust SIGNAL?

SIGNAL would rather flag an honest doubt than manufacture a false certainty.
  • It says when it is not sure

When SIGNAL cannot confirm that a record was saved or that a run started, the audit says so, instead of claiming that it did.

  • It recommends, it does not diagnose

Clinical flags are labeled "clinical review recommended" and "provider decision required", never presented as a confirmed diagnosis.

  • It stops instead of inventing

When a step needs a value that only your installation can supply, SIGNAL stops and flags it instead of inventing it.

A system that knows its limits is a system compliance can trust.

How does SIGNAL keep patient data safe?

  • SIGNAL is client-hosted: it runs inside your organization's environment, and no PHI is retained on third-party infrastructure.
  • The model that reads clinical notes runs locally inside your environment, with no network calls.
  • SIGNAL is designed to run air-gapped, with model updates delivered as signed packages that your administrator approves.
  • Every decision is recorded in an audit trail that stays inside your environment.
  • Data at rest is encrypted.
  • The provider always decides. SIGNAL has read-only access to your EMRs and does not code, confirm, or write to the chart.
  • SIGNAL reads your EHR through SMART on FHIR, read-only, using the minimal scopes your IT team provides and can revoke at any time.
  • An installed SIGNAL makes no outbound connections. It never calls home.
  • [PENDING: HIPAA/BAA wording, pending legal review]

Patient and clinical information handled by SIGNAL never leaves your organization's environment.

How does SIGNAL connect to our EHR?

Your IT team provides the tokens and URLs for local access to your EHR, and that is the only way SIGNAL reads data. There is no external communication.

Your organization keeps control at all times: SIGNAL can read only what your IT team grants, and your IT team can take that access back whenever it decides.

What does SIGNAL not do?

  • SIGNAL does not code or confirm diagnoses.
  • SIGNAL does not write to any EMR.
  • SIGNAL does not predict future conditions.
  • SIGNAL is client-hosted, not a multi-tenant cloud product.

Frequently asked questions

What is HCC risk adjustment software, and where does SIGNAL fit?
HCC risk adjustment software helps Medicare Advantage organizations make sure the conditions their patients have are documented and supported by evidence. SIGNAL reads charts and clinical notes and suggests the conditions the evidence supports, for the provider to review. It does not code or confirm diagnoses.
Does SIGNAL work with the CMS-HCC V28 model?
Yes. SIGNAL compares the clinical evidence already in your records against the CMS-HCC V28 model. It surfaces conditions the evidence supports but the current year does not document.
What is MEAT validation, and how does SIGNAL use it?
MEAT stands for Monitor, Evaluate, Assess, Treat. SIGNAL validates every finding against MEAT and shows the evidence behind it, so the provider and your compliance team can see why it was suggested.
How does SIGNAL help with RAF capture?
SIGNAL finds conditions a patient demonstrably has that were not documented this year, so they no longer fall out of risk adjustment unnoticed. Each one reaches the provider as a recommendation with its evidence, and the provider decides.
Does SIGNAL help us prepare for Medicare Advantage audits?
In a CMS audit, what counts is the medical record documentation behind each diagnosis. SIGNAL puts that evidence in front of the provider before they document, and keeps an audit trail of every finding inside your environment. When SIGNAL is not sure, it flags the doubt for review instead of presenting it as certain.
How does SIGNAL support clinical documentation integrity?
SIGNAL puts the evidence already in the chart in front of the provider, labeled as a recommendation, never as a diagnosis. The provider always decides, and SIGNAL never writes to the chart.
What access does SIGNAL need to our EHR?
SIGNAL reads your EHR through SMART on FHIR, read-only, with the minimal scopes your IT team provides. Your IT team can revoke that access at any time.
Does PHI ever leave our environment?
No. SIGNAL runs inside your organization's environment, and an installed SIGNAL makes no outbound connections. Patient and clinical information stays with your organization.
